Once you have reviewed the Level Up Participation Waiver & Release, please complete the form below to officially acknowledge the waiver and our Family Handbook.
The form will also collect your family information, emergency contact information, birthday information for Family Huddle celebrations, and photo permission.
One form per family. You will only need to complete this once for the 2026–2027 school year.
By completing the form, the parent or legal guardian will provide an electronic signature acknowledging the Family Handbook and Participation Waiver & Release.
